DEEP VEIN THROMBOSIS - ARCHIVE

Definition

Deep-vein Thrombosis (DVT) is defined as an occlusion of the venous system. Code only DVT’s below the diaphragm. Code only DVT’s confirmed by Doppler, ultrasound or venogram. If DVT is confirmed to be present, code the location and whether the patient received treatment with IV Heparin, Coumadin or a Filter.

RESPIRATORY FAILURE -ARCHIVE

Definition

Requiring ventilation greater than one week

Details

If patient is chemically paralyzed and on mechanical ventilation for greater than one week, code as respiratory failure present.

If patient is taken off ventilation, but failed extubation and put back on vent, add days on vent. together to determine if ventilation greater than one week, only if failed extubation is less than 48 hours in duration.

Patient is considered “off ventilation” when he/she is extubated.

If patient is on mechanical ventilation, it is coded as respiratory failure present, regardless of reason for ventilation.
